Caremark Home Health Care LLC is a licensed 254D provider of various services such as Adult Companion, Homemaker, Night Supervision, Personal Support, Respite, ILS with/without Training, In-Home Family Support, and Caregiver Living Expense. We have skilled Direct Support Staff members available to assist with individuals’ needs as required. We prioritize person-centered planning, ensuring tailored care to meet individuals’ unique needs and preferences. Additionally, our staff members are well-trained and supervised by the 245D coordinator, ensuring high-quality care provision

Adult Companion Services

Provide non-medical care, supervision, and socialization for individuals over the age of 18. This program helps individuals gain more independence by building their skills and reducing isolation, while also teaching them how to participate in the community and cope with anxiety.

Homemaker Services

Range from light household cleaning and laundry to assistance with home management and activities of daily living. Home management includes meal preparation, shopping for food, clothes, and supplies, household repairs, and arranging for transportation. Activities of daily living include bathing, toileting, grooming, eating, dressing, and walking assistance. Homemaker Services can be paid for through Medical Assistance, Home and Community Based Services waiver programs, or private pay.

Personal Support Services

Offer non-medical care, assistance, and supervision in the individual’s home or community. The service focuses on providing opportunities for the individual to interact with people with and without disabilities, with the goal of increasing independence and involvement in community activities.

Night Supervision Services

Provide overnight assistance and monitoring in the individual’s own home by an awake staff member. This service helps reinforce independent living skills, monitor safety, and assist with instrumental activities of daily living.

Caregiver Living Expenses

Reimburse certain expenses, such as rent and food, incurred by a caregiver who is living in the same household as an individual receiving waivered services and providing an approved service.

ILS Training Services help develop, maintain, and improve the community living skills of an individual through direct training by a Direct Support Staff member. This training addresses the individual’s skill development needs in communication, community living and mobility, interpersonal skills, challenging behavior reduction, self-care, and sensory/motor development.

In-Home Family Support provides assistance to develop, maintain, or improve skills related to activities of daily living for individuals and their families. This service can include training the person and family members to increase their capabilities to care for and maintain the individual in the home.
